How IVF Helps Couples Who Cannot Conceive Naturally

What medical services protect reproductive health?

Reproductive health also depends on safe medical care when problems arise — ending a pregnancy safely when needed, detecting disorders before birth, preventing and treating infections, and helping couples who cannot conceive.

What is medical termination of pregnancy, and why is it safest in the first trimester?

Medical termination of pregnancy (MTP) is the intentional or voluntary ending of a pregnancy before full term; India permits it under strict legal conditions to prevent misuse, and it is considered relatively safe only in the first trimester, up to about 12 weeks, because later procedures are much riskier.

Why MTP may be needed:

- To end unwanted pregnancies after unprotected intercourse or failure of contraception, or pregnancies resulting from rape
- When continuing the pregnancy would be harmful or even fatal to the mother or the foetus

The legal framework:

- Permits MTP only under specified conditions, performed by registered medical practitioners in approved facilities
- Aims to reduce illegal, unsafe abortions by unqualified people, which can be fatal

Why the first trimester:

- Up to 12 weeks, the procedure is comparatively simple and safe
- Second-trimester terminations carry much higher risk for the mother

An everyday example. Government hospitals provide MTP services confidentially, so that women do not turn to unsafe, unqualified providers.

The substance. MTP is not a method of contraception — it is a medical procedure for specific situations, not a substitute for preventing pregnancy.

What is amniocentesis used for, how has it been misused, and why is sex determination banned?

Amniocentesis tests the amniotic fluid surrounding the foetus to detect chromosomal and genetic disorders, but it has been misused to find the sex of the foetus so that female foetuses could be aborted, which is why using it for sex determination is banned by law.

How it works:

- A small amount of amniotic fluid is withdrawn from the sac around the foetus
- The fluid contains foetal cells and dissolved substances
- These are analysed for chromosomal abnormalities, such as Down's syndrome, and some genetic and metabolic disorders

Misuse and the ban:

- Because foetal cells reveal the sex chromosomes, the test has been misused to identify female foetuses for abortion
- Such misuse leads to female foeticide and an unbalanced sex ratio
- Indian law bans prenatal tests, including amniocentesis, for sex determination, while allowing them for detecting genuine disorders

An everyday example. Notices in clinics and hospitals stating that sex determination is illegal are a direct result of this law.

The substance. The technique itself is not banned, only its misuse — amniocentesis remains a valuable diagnostic tool.

What are the common sexually transmitted infections, their symptoms and complications, and how can they be prevented?

Sexually transmitted infections (STIs) such as gonorrhoea, syphilis, genital herpes, chlamydiasis, genital warts, trichomoniasis, hepatitis-B and HIV spread mainly through sexual contact; early symptoms are often mild or absent, untreated infections can cause serious harm including infertility, and safe practices prevent most transmission.

The infections:

- Gonorrhoea, syphilis and chlamydiasis — caused by bacteria
- Genital herpes, genital warts, hepatitis-B and HIV — caused by viruses
- Trichomoniasis — caused by a protozoan
- Except for hepatitis-B, genital herpes and HIV, these are completely curable if detected early and treated properly

Other routes. Hepatitis-B and HIV also spread through shared injection needles, transfusion of infected blood and from an infected mother to the foetus.

Early symptoms include itching, fluid discharge, slight pain and swelling in the genital region.

Complications if untreated:

- Pelvic inflammatory disease (PID)
- Abortions, still births and ectopic pregnancies
- Infertility
- Cancer of the reproductive tract

Prevention:

- Avoid sex with unknown partners or multiple partners
- Always use condoms during coitus
- In case of doubt, see a qualified doctor for early detection and complete treatment

An everyday example. Blood banks test every donated unit for HIV and hepatitis-B before it is used for transfusion.

The substance. Having no symptoms is not the same as being safe — many STIs cause no early signs yet can still lead to infertility.

What causes infertility, and how do IVF, ZIFT, GIFT, ICSI and artificial insemination help?

Infertility — the inability to conceive despite unprotected sexual cohabitation — can have physical, congenital, disease-related, drug-related, immunological or psychological causes in either partner, and assisted reproductive technologies help by bringing gametes or embryos together inside or outside the body.

Assisted reproductive technologies (ART):

- IVF with embryo transfer — ova and sperm, from the couple or from donors, are fertilised in the laboratory, and the resulting embryo is transferred into the woman
- ZIFT — a zygote or early embryo with up to 8 blastomeres is transferred into the fallopian tube
- IUT — an embryo with more than 8 blastomeres is transferred into the uterus
- GIFT — an ovum from a donor is transferred into the fallopian tube of a woman who cannot produce ova but can support fertilisation and development
- ICSI — a sperm is injected directly into the ovum in the laboratory
- Artificial insemination — semen from the husband or a healthy donor is introduced into the vagina or, in IUI, into the uterus; used when sperm counts are low or insemination is not possible

Social relevance:

- ART needs highly skilled professionals and expensive equipment, so it is costly and available only in a limited number of centres
- Legal adoption is another way to have a family while giving a home to orphaned and destitute children

An everyday example. Fertility clinics in many Indian cities offer IVF, but its cost keeps it out of reach for many couples.

The substance. Infertility is not always the woman's problem — causes are often found in the male partner too.
