Why Reproductive Health Is About Much More Than Having Children

Understand why reproductive health matters and the national programmes that promote it, how methods of contraception work, what medical termination of pregnancy, amniocentesis and assisted reproductive technologies are, and how sexually transmitted diseases can be prevented.

What does reproductive health really mean?

Reproductive health means total well-being in every aspect of reproduction — physical, emotional, behavioural and social.

This lesson covers the need for reproductive health and national programmes, contraception, MTP, amniocentesis and assisted reproduction, and sexually transmitted diseases.

Why is reproductive health important, and what do India's reproductive-health programmes do?

Reproductive health matters because it lets people make informed, safe choices, protects them from infections, lowers maternal and infant deaths and supports healthy families, and India promotes it through nationwide family planning and Reproductive and Child Health (RCH) programmes.

Why it is needed:

- Correct, age-appropriate information about reproductive organs, puberty and safe practices
- Awareness of sexually transmitted infections and how to prevent them
- Care for mothers during pregnancy and childbirth, and for newborn babies
- Informed choices about the number and spacing of children

National programmes:

- Family planning programmes spread awareness of contraception and small, healthy families
- Reproductive and Child Health (RCH) programmes cover a wider range, including safe motherhood, child immunisation and prevention of infections
- A legal ban on using amniocentesis to find the sex of a foetus helps prevent female foeticide

An everyday example. ASHA and anganwadi workers in villages counsel families on antenatal check-ups, immunisation and spacing between children.

The substance. Reproductive health is not only about preventing births — it equally covers safe pregnancy, child health, infection prevention and help with infertility.

What are the methods of contraception, and how does each one work?

Contraceptive methods prevent pregnancy by stopping sperm from meeting the ovum, by preventing ovulation or implantation, or by permanently blocking the gamete ducts, and an ideal method is user-friendly, easily available, effective and reversible with few side effects.

Natural methods:

- Periodic abstinence — avoiding intercourse around the middle of the cycle, when ovulation is likely
- Withdrawal and lactational amenorrhoea, which relies on the absence of ovulation during intense breastfeeding — both fail often

Barrier methods. Condoms, diaphragms, cervical caps and vaults physically stop sperm from reaching the ovum; condoms also help protect against sexually transmitted infections.

Intrauterine devices (IUDs):

- Copper-releasing IUDs, such as CuT and Cu7, release copper ions that reduce sperm motility and fertilising capacity
- Hormone-releasing IUDs make the uterus unsuitable for implantation and the cervix hostile to sperm

Hormonal methods:

- Pills of progestogens or progestogen-oestrogen combinations inhibit ovulation and implantation and alter cervical mucus
- Injections and implants under the skin work in a similar way for longer periods
- Emergency contraception — hormonal pills or an IUD within 72 hours of unprotected intercourse

Surgical methods:

- Vasectomy — a small part of each vas deferens is removed or tied
- Tubectomy — a small part of each fallopian tube is removed or tied

An everyday example. Government health centres across India provide contraceptives free of cost, so safe family planning is available to everyone.

The substance. Only barrier methods such as condoms also protect against sexually transmitted infections — pills, IUDs and sterilisation do not.

What are medical termination of pregnancy, amniocentesis and assisted reproductive technologies?

Medical termination of pregnancy (MTP) is the intentional ending of a pregnancy before full term under legal and medical safeguards, amniocentesis tests foetal cells in amniotic fluid for genetic disorders, and assisted reproductive technologies help infertile couples conceive.

MTP:

- Permitted in India under the MTP Act, within set conditions and with the opinion of registered medical practitioners
- Considered relatively safe during the first trimester, up to about 12 weeks of pregnancy
- Unsafe abortions by unqualified people are dangerous and can be fatal

Amniocentesis:

- A small sample of amniotic fluid is drawn to study foetal cells and chemicals
- Detects chromosomal disorders such as Down syndrome
- Using it to find the sex of the foetus is banned by law in India

Assisted reproductive technologies:

- IVF-ET — ova and sperm are fertilised in the laboratory and the embryo is transferred
- ZIFT — a zygote or early embryo with up to 8 blastomeres is transferred into the fallopian tube
- IUT — an embryo with more than 8 blastomeres is transferred into the uterus
- GIFT — an ovum from a donor is transferred into the fallopian tube of a woman who cannot produce ova but can support fertilisation
- ICSI — a sperm is injected directly into an ovum
- Artificial insemination — semen is placed into the vagina or uterus when the male partner has a low sperm count

An everyday example. Infertility clinics in Indian cities offer IVF and ICSI to couples who cannot conceive naturally.

The substance. Infertility is not always a problem with the woman — it can arise in either partner, which is why both are tested.

What are the causes, symptoms and prevention of common sexually transmitted diseases?

Sexually transmitted diseases (STDs), also called sexually transmitted infections, are caused by bacteria, viruses or protozoa passed on mainly through sexual contact, and most can be prevented by safe practices and treated if detected early.

Common STDs and their causes:

- Gonorrhoea, syphilis and chlamydiasis — bacteria
- Genital herpes, genital warts, hepatitis B and HIV infection — viruses
- Trichomoniasis — a protozoan

Other routes. Hepatitis B and HIV can also spread through shared injection needles or surgical instruments, transfusion of infected blood, and from an infected mother to the child.

Symptoms. Early signs can include itching, fluid discharge, slight pain and swellings in the genital region, though some infections cause no symptoms for a long time.

Prevention:

- Avoiding sexual contact with unknown or multiple partners
- Using condoms during intercourse
- Consulting a qualified doctor early for diagnosis and complete treatment

An everyday example. Blood banks in India screen donated blood for HIV and hepatitis B before use, to stop these infections spreading through transfusion.

The substance. Hepatitis B, genital herpes and HIV cannot be completely cured — unlike most other STDs, which respond to early treatment, so prevention matters most.

Classify every contraceptive by how it works — natural, barrier, IUD, hormonal or surgical — and give one example for each class.

- Copper IUDs reduce sperm motility; hormonal pills inhibit ovulation and implantation
